Changes to your overall health can impact your dental health. All patients must keep us informed about their overall health and the medications they are currently taking. Be sure to bring a list of all medications you are taking, including dosages (this includes over-the-counter medications and natural health products), to your first visit. Please continue to update us as your medications change.
DENTAL HISTORY
If you have been attending regular dental visits, please contact your previous dentist to see if there are any current X-rays that can be forwarded to our office. If you prefer, please call us, and we will be happy to assist you.

We will review three key areas during your first visit to ensure we have a clear understanding of your oral health and your specific needs.
A REVIEW OF YOUR DENTAL CARE HISTORY
We will review your dental care history and address any concerns you currently have about your oral health.
A COMPLETE ORAL EXAM WITH X-RAYS
We will conduct a comprehensive oral examination, including X-rays, to assess the overall health of your smile.
A FULL ORAL HEALTH PLAN
We will prepare recommendations for your overall oral health, which will include comprehensive oral maintenance, as well as potential additional dental services that we agree upon.

IMPORTANT INFORMATION ABOUT INSURANCE BENEFITS
Remember that you are an important part of your dental care and that treatment and care decisions should be made in collaboration with your dental care provider.
A dental plan can be a valuable aid in covering the costs of dental care. However, it is possible that your dental plan may not cover the full cost of your required treatment. By maintaining good oral care habits in your daily life, you are making a long-term investment in your well-being.
Balsam Dental is pleased to submit, process, and complete any necessary forms or estimates at your request; however, dental insurance is a contract between you and your insurance carrier. Therefore, it is your responsibility to know what your insurance covers and to inform our office of any changes to your insurance, such as the carrier, policy, identification numbers, or plan maximums. This will ensure that your claims are processed correctly.
Our practice will assist patients with most dental plans and help our clients receive reimbursement from their insurance programs. If you have concerns about your insurance benefits, please don’t hesitate to call our office and speak with our treatment coordinator.
CHANGING APPOINTMENTS
Your dental appointments are reserved specifically for you. Although we will contact you before a scheduled appointment with a courtesy reminder, it is ultimately your responsibility to ensure that you arrive on time for the appointment.
If you need to reschedule a reserved appointment, we require a minimum of 2 full business days’ notice. This permits another patient to receive dental care in your absence. If you miss an appointment without providing notice, a missed appointment fee will be applied.
PAYMENT
Patients are responsible for paying their dental procedure fees at the time of services. We accept the following payment methods.
